  5. A statement that the information in the notification is accurate, and under penalty of perjury, that you are authorized to act on behalf of the owner of an exclusive right that is allegedly infringed.
  6. Your physical or electronic signature.

Upon receipt of a valid copyright infringement notice, we will take appropriate action, which may include removing or disabling access to the allegedly infringing material, notifying the user responsible for the material, and/or terminating the user’s account.

Counter-Notification:

If you believe that the material removed or disabled as a result of a copyright infringement notice is not infringing or that you have the authorization from the copyright owner, the copyright owner’s agent, or pursuant to the law, you may send a counter-notification to our designated copyright agent.

Your counter-notification must include the following information:

  1. Identification of the material that has been removed or to which access has been disabled, and the location at which the material appeared before it was removed or disabled.
  2. A statement under penalty of perjury that you have a good faith belief that the material was removed or disabled as a result of mistake or misidentification.
  3. Your name, address, telephone number, and email address.
  4. A statement that you consent to the jurisdiction of the courts located within your jurisdiction and a statement that you will accept service of process from the person who provided the original copyright infringement notification or an agent of such person.
  5. Your physical or electronic signature.

We reserve the right to remove or disable access to any material on survivevanlife.com that is claimed to be infringing or to be the subject of infringing activity, and to terminate the accounts of repeat infringers, in accordance with applicable copyright laws.
